Test form without data capture
The proposal form does not transmit, store or synchronise data. Use test data only. There is no account, CRM, clinical record, local storage or response server implemented for the form.
Local review annotations
Annotation mode saves markers and comments only in this browser’s storage when available. It does not send notes to the team or synchronise between phones, computers or owners. Export to back up and share through an agreed channel. Clearing storage, private browsing or switching devices can lose notes. Do not enter passwords, clinical data or patient information. Author names are local labels, not verified accounts. Resolving a note does not apply changes or approve content.
External services
Loading the map connects the browser to Google. External links lead to services with their own policies. The review tool does not transmit comments; hosting may log access and its visit analytics are enabled. This environment does not promise complete anonymity or an absence of technical logs.
Before enabling the final site
Define the data controller, purposes, applicable basis and consent, retention, rights channel, providers and security. Separate the administrative form from the clinical record. This demonstration text is not presented as the clinic’s legally approved policy.
